Your First Visit to Our Wilmington Practice

We Welcome New Patients!
Your initial appointment will consist of a consultation, necessary x-rays, professional cleaning, and a gathering of information concerning your existing dental and medical conditions. We’ll schedule a subsequent comprehensive examination appointment with Dr. Mazoch to explain your diagnosis and treatment options. You may also fill out new patient forms online prior to your first visit.
Please assist us by providing the following information at the time of your consultation:
- Records and x-rays from your previous dental care provider.
- List of medications, including herbals, your physician’s name and contact information.
- Medical conditions, such as diabetes, high blood pressure, heart conditions, etc.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Insurance Information
If you’re planning to use insurance, please be sure to bring your card with you. As a courtesy, we will file the necessary benefit forms with your insurer in order to maximize your reimbursement. If you have questions regarding your coverage or plan, we’d love to help. Please contact us and we’ll make every effort to make sure you understand your benefits and responsibilities.
Patient Billing
Please note that your payment is due at the time service is rendered. For your convenience we accept Visa, MasterCard, Discover, CareCredit and CitiHealth, trusted third-party medical financing. Our financial coordinator will be happy to discuss your method of payment with you.
Available Discounts
Ask us about our dental hygiene on-call club for patients without insurance who are able to take appointments within a very short notice. We offer 20% off the fee for your professional cleaning if you pay with cash or a check and 15% off of your professional cleaning when you pay with a credit card.
We also offer a 5% discount on treatments over $1,200 when the fee is paid in full with a check or cash prior to the procedure. Please feel free to ask about this at the time you schedule your appointment.

Donated Dental Services
Donated Dental Services is a division of the National Foundation of Dentistry for the Handicapped. Dr. Mazoch is a volunteer dentist member. She sees several patients each year who are either physically or mentally handicapped, elderly, or medically compromised and who are not eligible to receive public assistance but cannot afford treatment. Qualifying patients receive comprehensive care free of charge. If you or someone you know is in need, please check out the DDS program.
